- Contact Our Care Team: Reach out to OmaCare to discuss your needs and hospital discharge plans. This can be done prior to your loved one leaving the hospital
- Care Assessment: A Care Manager will visit or consult virtually to assess health, mobility, and home environment.
- Personalized Care Plan: We create a tailored plan outlining services, schedules, and goals for recovery.
- Care Begins: Our team starts 24–48 hour post-discharge support and continues monitoring, adjusting the plan as needed.
